How do I change my beneficiary?

To change your beneficiary you must know what MERS plan(s) you are enrolled in. Next, complete the form specific to your plan (see below) and mail or fax to MERS, along with supporting documentation (i.e. Judgment of Divorce, Marriage License). If you are enrolled in multiple MERS programs, you will need to submit a separate beneficiary form for each one because definitions and requirements for beneficiaries differ for each program.
