How do I change my name and/or address?

To update your address and contact information, please log in to your personal account, then go to "myProfile," click on the "myInformation" tab, and select "Update Permanent Address" from the Actions menu. You can also change your contact information by simply calling 800.767.2308 or by sending us a completed Personal Information Form. 

If you are changing your name, you will need to submit one or all of the following as they apply to your situation: a copy of your marriage certificate, a complete copy of your judgment of divorce, or any other legal court documentation required to make this change. This documentation must be attached to a completed Personal Information Form at the time of submission, otherwise your request will be held until our receipt of this required information.
